Of the myriad of reasons our patients share as a need for orthodontic care, fixing an overbite or underbite are two of the most common reasons expressed. Many people choose to fix an overbite and underbite for purely aesthetic reasons. However, in some some extreme cases, they can cause serious health issues, such as speech impediments, trauma to the teeth, difficulty chewing, gum damage, jaw pain, and more.

The Condition and Cause of Overbites and Underbites

An overbite occurs when the upper teeth cover the lower teeth — also known as an “overjet.” It generally occurs as a result of genetics, bad oral habits, or overdevelopment of the bone that supports the teeth. Overbites can lead to gum irritation and issues caused by wearing on the lower teeth unnaturally. It can also cause painful jaw and joint problems.

Conversely, underbites occur when the lower teeth extend out beyond the upper teeth, often making the lower jaw more prominent. This alignment occurs by the overgrowth of the lower jaw, undergrowth of the upper jaw, or both in some cases. Underbites prevent normal functions of the mouth from happening, including front teeth and molars, which can lead to painful tooth wear, joint issues, and limited function.

How Braces and Invisalign Work

Here are the basic ways in which Invisalign and braces work to straighten your teeth:

Braces

Braces use constant, gentle pressure to move your teeth into their proper positions over time. The two main components of your braces include: 1) brackets on your teeth and 2) a main archwire to connect them. We first affix the brackets — clear or metal — to each tooth and then thread the wire threads through the brackets to move your teeth into ideal alignment.

Invisalign

Without the use of wires and brackets, Invisalign straightens your teeth with a series of clear, customized, removable aligners. Every one to two weeks, we design a new set of aligners to gradually move your teeth into proper placement. Along the way, patients enjoy the ability to easily remove the aligners for eating, brushing and flossing, without calling unwanted attention to their teeth.

5 Differences in Invisalign vs Braces

Now that you understand the basics, let’s dive into the details and compare the specific differences between Invisalign and braces:

  1. Materials: Braces consist of metal or ceramic brackets and archwires, as well as rubber bands, whereas Invisalign consists of a BPA-free transparent plastic material.
  2. Cost: In our offices, there is no cost difference between braces and Invisalign, although many other offices charge more for Invisalign than braces.
  3. Follow-up Visits: When necessary, braces require that you see your orthodontist about every 6-8 weeks for follow-up visits to check on your progress and adjust wires. Invisalign consists of a series of custom-made aligners and a virtual treatment plan, which means far less check-up appointments, usually every 12 weeks.
  4. Convenience: Braces can be quite difficult to maintain at times, with many food restrictions and complex cleaning routines. Patients must also follow a long list of foods to avoid with braces. On the other hand, patients can simply take out Invisalign aligners before eat any food they desire. Invisalign requires you to rinse your aligners in warm water whenever you take them out.
  5. Duration: In compliant patients, there is no difference in treatment time between braces and Invisalign. In non-compliant patients, traditional braces can require slightly less time to complete treatment, since the brackets are permanently fixed to the teeth. With Invisalign, while we recommend wearing the aligners for at least 22 hours a day, patients may prolong the treatment schedule by wearing them less than we suggest.

To make your Invisalign process as easy and successful as possible, consider following these six tips for a better experience.

1. Use Your Invisalign Case to Store Your Aligners

Keep your aligners in their case to prevent losing them, throwing them away, or allowing easy access for your dog to turn them into a chew toy. Additionally, cases will protect your retainers from dirt and germs found in bags, pockets, tables, etc.

2. Brush Your Teeth After Meals and Snacks

Brushing after every meal or snack will prevent your aligners from smelling and tasting like food. And remember, never eat or drink anything but water with your aligners still inside your mouth.

Many patients must prolong their treatment time because they forget to continually place their aligners back in after a meal or snack. We suggest setting a timer whenever you remove your aligners so you remember to put them back in whenever you’re done eating. This practice will ensure a timely treatment plan and quicker results.

5. Soak Your Trays While You Eat

Whenever you take your aligners out to eat, consider using that time to soak them in retainer or denture cleaner — preventing them from getting a bad odor or becoming stained. Twenty minutes of soaking per day will keep your aligners clean and smelling fresh.

6. Remove Your Trays While Working Out

Running, playing sports or doing intense cardio while wearing your Invisalign trays can be very difficult because of the dryness or spit that accumulates in your mouth. We recommend taking them out while you workout, as long as you remember to put them back in afterwards.

Family Life, Hobbies and Goals

Sherry enjoys reading and playing volleyball and basketball, as well as volunteering in the church nursery every Sunday. One of her personal goals is to not be shy around people and try new things with courage. Sherry also wants to go to college to be a dentist or nurse. As she describes it, “it’s a hard choice.”

Sherry has four siblings, two older brothers and two younger brothers. They all like to watch NBA and NFL games together, as well as play musical instruments.

How Receiving Orthodontic Treatment Can Change Her Life

In Sherry’s words: “I need braces because my teeth aren’t growing normally. I have a huge gap when I shut my teeth and am in danger of having a speech impediment if I don’t get braces soon. I will remember Gorman & Bunch Orthodontics forever in my life when I get treatment. I’ll be a happier person, be smiling publicly, and be able to pronounce words correctly.”

Paying it Forward Through Volunteer Work

Sherry will be volunteering with Open Doors — a local food pantry for families who need food and clothings. Their mission is to “improve the quality of life for Washington Township residents through food and clothing assistance, referral to community services, and spiritual guidance. The organization will be moving locations soon and Sherry will be helping them stock food and clothes. She may also work in the media department, which helps raise money to buy food for the pantry, and she’s also interested in serving in the education department, which helps spread the word to churches and local grocery stores on ways to donate.

Per Sherry: “Open Doors is counting on me, so I will always try to be on time and do the work right. The most important thing about helping is knowing that I’m helping families and individuals who are not able to afford food and clothes. I want to be a part of a caring and helping community.”

One of the winners, Samantha Sager, now attends Purdue University, starting her freshman year as a pre-pharmacy major. We recently sat down and talked with Samantha. Meet the First Gorman and Bunch Smile for a Lifetime Recipient

Can you talk a little about your upbringing?

I grew up on the southside of Indianapolis, the second youngest of six kids. My father passed away when I was three years old, and my mother raised us all on her own. Around the time I was six or seven, she met a man who was involved with the wrong crowd, and my home life became very unstable. As a result, my older siblings began moving out and started lived with friends. By the time I was a freshman in high school, I knew I needed a better environment to really concentrate and be successful in school and in activities. I moved out at the age of 15 and lived with my sister for a short while in Lebanon until she decided to pursue her military career in Alabama. After she moved I stayed with a friend of hers and then eventually moved in with my boyfriend’s parents.

How did you first discover the Smile for a Lifetime program, and what does it mean to you?

I first discovered Smile for a Lifetime after I came in for a free consultation with Heather Edwards at Gorman & Bunch Orthodontics, I knew that I would most likely not be able to afford it, even though I’d been working two jobs and saving money. After I told Heather about my situation she suggested that I apply for the scholarship, and I’m so glad I went in for that consultation!

Why is orthodontic treatment so important to you? What do you think a better, healthier smile might grant you?

I’ve always been very insecure about how my teeth look, oftentimes putting myself down because of it. Growing up, I was bullied at school and sometimes by my siblings, who were lucky enough to be born with a perfect smile. I wanted braces so badly that I would ask family members to get them for me for Christmas or my birthday—even pleading with that cheesy line, “but I’ll never ask for anything ever again!” I’ve never really had that much confidence about my teeth, but I have seen it increase significantly ever since I started treatment. Even though I have a long way to go, I know that I will soon have a healthier, prettier smile, and that makes me so happy!

Can you share about your “pay it forward” plan as part of the Smile for a Lifetime program?

I’ve volunteered all throughout high school and made some connections within my community that help me to pay it forward. For example, I’ve served food to the needy at the Shalom House in Lebanon and volunteered in the art department at the local Boys and Girls Club. I’ve also been part of many clubs that focus on giving back to the community, such as Key Club, Best Buddies, and the Thirst Project.

In college, I plan to be part of similar clubs that focus on making the community a better place for everyone. Then after I’ve finished school and paid off my debt, I plan to donate back to the organizations who helped me follow my dreams. I’ve already started a list of different outside scholarships I received for school and Smile for a Lifetime sits at the top. Although it’s not a school-related organization, Smile for a Lifetime is an amazing program that I hope sticks around forever to help those kids who can’t afford it—but deserve it!

What’s next? Where do you plan to go from here in your life?

I now attend Purdue University, studying pre-pharmacy. If everything goes as planned, I will be spending the next six years of my life in West Lafayette! After school, I plan to stay in Indiana and work at a retail pharmacy, unless a different opportunity sparks my interest.

Our Smile for a Lifetime program allows kids and teens in Indianapolis and the surrounding communities to submit their application for free orthodontic care—including full coverage of all digital scans, x-rays, appliances and all necessary treatments to straighten teeth and optimize oral health—and offers us a chance to live out our core values and serve the community on a deeper level.

In April 2017, the Smile for a Lifetime Board of Directors selected our first recipients for the program. We have been treating two Central Indiana teenagers over the past three months with braces and Invisalign, building their confidence to smile with each appointment.

Providing the Community With Exceptional Service

We currently operate the program out of our five offices in Carmel, Westfield, Kokomo, West Carmel/Zionsville, and Fishers. Anyone who is within reasonable driving distance—approximately one hour—of those offices can absolutely be part of this program. In 2017, we hope to serve more families by expanding the program to include our Marion and Wabash offices.

Who is eligible for Smile for a Lifetime?

Our Smile for a Lifetime Board of Directors meet three times a year to review applications and award up to six scholarships per term. Our board considers candidates who meet the following criteria:

  • Between the ages 11 and 19 years old.
  • Lives within one hour driving distance from our Carmel, Westfield, or Kokomo offices.
  • Does not exceed family income threshold of 200% of the Federal Poverty Level Annual Gross.
  • Has a significant esthetic need for braces.
  • Agrees to follow the treatment plan, maintain good oral hygiene, and demonstrate the ability and commitment to make all appointments on time.
  • Currently enrolled student who maintains a minimum 2.0 GPA.
  • Willing to complete 40 hours of community service during the treatment period.
  • Shows involvement and leadership in extracurricular activities.

Gingivitis

Also heard in dental offices, gingivitis occurs when the fibrous gingiva tissue at the base of your teeth gets irritated, causing redness and inflammation. It’s a mild form of periodontal (gum) disease, but can lead to more serious oral concerns if not addressed.

– H –

Hard Cast Retainer

After moving all teeth into proper position, we may ask you to wear a hard cast retainer full time for 10 weeks. This clear retainer fits snugly over your teeth and holds them securely into position while the adjoining bone matures. This critical step can help achieve a stable result that will last. We always advise our patients to wear a retainer that fits even years after treatment, so it’s vital to let us know immediately if your hard cast retainer ever breaks, gets lost, or fits poorly.

Headgear

Everyone’s favorite orthodontic appliance, the headgear includes an external wire known as a facebow that gently guides the growth of your face and jaw by moving your teeth into the correct position. The device applies force to the facebow by a spring-loaded neck strap or head straps, which have a safety release that disconnects if the facebow is pulled or snagged.

Herbst Appliance

This appliance is used to correct skeletal imbalances where the lower jaw is behind the upper jaw. A combined restriction of upper jaw growth with an enhancement of lower jaw growth aids in the correction process. The Herbst appliance is generally worn between 12-15 months.

Hook

A hook is a welded or removable arm on your braces bracket where we attach the elastics.

Impacted Tooth

The term “impacted tooth” encompasses any tooth that has not yet emerged, does not have adequate space to grow, or is poorly positioned in your mouth and needs help coming in.

Impressions

This is the first step in making a model of your teeth for creating personalized orthodontic appliances. They are made by seating into a container filled with a rubber-like material over your teeth. That material then hardens to produce a mold of your teeth, which we examine to create the proper orthodontic treatment plan for you. Not many people’s favorite process, but fortunately with our 3D digital scanners we have eliminated most traditional impressions from our offices!

Invisalign

Invisalign is a technology created using computer scanners and virtual reality to move teeth gradually using clear, customizable and removable appliances called aligners. Read more about our Invisalign treatment here.

Incisor

Positioned at the front of our mouths, the four incisors are used to cut through food.

Ligature Tie

Typically made out of elastic, the ligature is the elastic tie we use to attach the bracket to the archwire. (Look here for a refresher on what “bracket” and “archwire” mean.)

Lower Bonded Retainer

After treatment with Invisalign or braces, we oftentimes place a lower bonded retainer behind the bottom front teeth, glued to the lower canine teeth. This “permanent” retainer keeps your lower teeth straight over the long term, without the potential for loss or damage with a removable retainer. Proper brushing and flossing are extremely important to keep this type of retainer clean and keep your teeth and gums healthy.

Why You’re Likely an Ideal Candidate for Invisalign

We find that many Gorman & Bunch Orthodontics’ patients walk into our offices erroneously assuming they’re not going to be an ideal candidate for Invisalign treatment. That it was for the next patient, not for them. We’re not exactly sure where that idea originated, but it’s just not true—not in any of our Central Indiana Offices at least.

Since we first added the technique in the year 2000, we’ve been able to help more than 1,100 patients (as of the writing of this blog) in our Wabash, Marion, Kokomo, Westfield, Zionsville, Fishers, and Carmel offices—making us the top provider of Invisalign and Invisalign Teen in the state of Indiana. And we’ve actually treated more cases with Invisalign than 99% of doctors throughout the entire continent of North America.

How Does Invisalign Orthodontic Treatment Work?

Invisalign allows you to choose something other than braces to straighten your teeth. No metal brackets. No wires. Not even any messy impressions of your teeth.

Instead, we digitally scan your teeth to create a 3D model of your mouth that we then use to make your first set of aligners to fit precisely over your upper and lower teeth. As you wear the clear, plastic aligners for 22 hours a day—only taking them out to eat, brush, and floss your teeth—your teeth gradually shift toward proper alignment. Every one to two weeks, you switch to a new set of aligners until you receive that healthy smile you desire.

And because the Invisalign aligners are virtually invisible, you get to transform your smile without many people knowing.

Can Invisalign Aligners Fix Both Minor and Severe Issues?

The short answer is Yes. We’ve been able to use Invisalign on patients with everything from simple crossbites to severe crowding. It’s not just for minor conditions.

In fact, if you’re experiencing any of the following moderate to serious issues, Invisalign can most likely work for you:

  • Large spaces between teeth
  • Overbites and underbites
  • Upper and lower front teeth don’t meet (also known as open bite)
  • Upper front teeth covering too much of the lower front teeth (also known as deep bite)

Is Invisalign Affordable When Compared to Braces?

At Gorman & Bunch Orthodontics, Invisalign orthodontic treatment can cost the same as traditional braces because we choose not to include additional charges for Invisalign versus traditional braces—not always the case with Invisalign providers.

And so, for many of our patients, they get to choose between traditional braces and Invisalign based on what works best for their lifestyle than what costs less.
